  1. Are you currently working?
  2. Will the medical impairment last for 12 months or end in death?
  3. Does the medical impairment meet or equal a medical listing?
  4. Can the individual perform their past relevant work?
  5. Can the individual perform any other work within the national economy?

  • Medical records from your doctors, therapists, hospitals, clinics and caseworkers that you already have in your possession
  • Laboratory and test results
  • A summary of where you worked and the kind of work you did
  • A copy of your most recent W-2 Form (Wage and Tax Statement) or, if you are self-employed, your federal tax return for the past year; and Ten (10) years of employment history
